Performance Management
Assessment (Evaluation) is a process by which a supervisor reviews an employee's performance. A review of goals and plans are discussed for future performance. All forms are to be signed by employee and supervisor. Administrative and Supervisory EmployeesThe Human Resource Office will send out evaluation requests beginning in December. This process will continue thru March. Full-time ClassifiedMarch of each year, the Human Resource Office will send out evaluation requests for all staff who are on a regular evaluation schedule. Notification of 90 and 180 day evaluation will be sent to supervisors in order to evaluate probationary employees. Employees will receive the job description and the probationary form to review five days prior to the evaluation. Full-Time FacultyThe faculty evaluations process begins in August and is completed by March. Please review the FT Faculty Agreement. Part-Time FacultyIn January, Human Resources will send out a notification to PT Faculty members who can expect an evaluation over the ensuing year. The Associate Dean/Department Chair will then follow up prior to the specific term the evaluation information will be gathered. Below are the self evaluation and peer evaluation forms, please check in with your Associate Dean/Department Chair on which form is appropriate for you.
After the self and peer evaluation information is gathered, the Associate Dean/Department Chair will summarize your performance feedback using the below rubric and form. The summative form will then be placed in your personnel file with Human Resources. |
