Payroll: Overtime Requests

There is one standardized form for overtime requests. (See Forms under the Human Resource website) If any other type of form is used, it will be returned which could delay payment.

Only one overtime form should be turned in each month for an employee.

All parts of the form must be completed:

  • department name
  • fiscal year
  • employee name
  • social security number
  • month of activity
  • days of overtime
  • account number
  • hours
  • purpose of overtime hours
  • The employee and supervisor must sign and date the form.

Send form by the 10th of the month to Sara Simmons in Human Resources for processing.
